病人參與醫療可以有很多種型態,日本在早年就有一句比較特殊的稱呼叫作「患者參加型醫療」=Patient participatory medicine,但在歐美諸國比較常聽到的同義語包括patient engagement, patient involvement, patient-centered care, patient and family-centered care , person and family engagement。
其實以上這些,都是指我們病人安全同好會(TPSCC)目前積極籌組中的倡議團體PFPS (Patients for Patient Safety, TW)想要做的事與達成的目標。
病人若能主動參與病安活動,在醫療照護的過程中不僅能提升病人安全,確保預後良好,對醫護人員來說,也可營造雙贏的醫病關係。
在這樣的互動角色裡,需要把醫療團隊的成員視為伙伴,尤其希望能讓病人真正成為積極主動,知情或有被告知的一員(Active member)。病人參與醫療,可依其參與的程度不同,分為幾個層次:
「知情抉擇」是由醫師提出不同方式的檢查或治療,並同時分析各方式的優缺點,或由大眾媒體提供給病人想要去就醫的醫院,或醫療專家讓病人自己選擇。
「共同抉擇=SDM」是病人在具有相當水準的醫療知識下,與醫師共同決定如何進行醫療,這是達到病人安全及提升醫療品質的最高理想目標。
(四)經由學習充實自己以求自保、主動接受衛教:透過網際網路學習常見疾病的訊息,例如PatientAider運用各種病患用的查核表可預知風險,又如跌倒風險的自我評估也是。此外,民眾可參與醫院舉辦之救護訓練,例如:心肺復甦術(CPR),藉以提升救命成功率。
(七)為緊急狀況做好防災準備:住院時,可先熟悉環境,以預防災難(火災、地震)來臨時,能配合緊急疏散與避難。
(十一)利用通暢的申述管道及通報系統,指出或提出不當醫療行為並通報異常:民眾亦可通報給院方高層、衛生單位及醫策會等,以利檢討改進,必要時也可應邀參加根本原因分析RCA會議。
