衛生署長楊志良新官上任第一天，即毫不客氣地宣布了健保調漲費率的計畫，一個月內即將報院。楊志良敢捅這個馬蜂窩，應該是有備而來；問題在，他得讓民眾看看他對健保的通盤改革方案，才知道他的衝勁值不值得支持。
對於楊志良的態度，外界有兩種不同的反應。一種是認為他缺乏官場經驗，所以一下子衝過了頭，簡直是自殺行為；另一種則是欣賞他的直率，認為他直搗健保問題核心，為人所不敢為，是有擔當的人。直率是台灣官場日益缺乏的品質，值得鼓勵；但勇敢畢竟不同於魯莽，不管楊志良多麼敢衝，也不能不講究溝通。尤其，健保費率牽一髮動全身，所有民眾都被波及，這不是他一個人當「烈士」就能完成的任務。
台灣的健保制度在國際上備受好評，因為它兼含社福意味的保險設計，使得廣大民眾乃至貧弱階層都能受到照拂，這樣的制度當然需要好好珍惜與保養。但不可否認，健保開辦十四年來，制度中一直有許多問題需要克服與改善，卻始終不見成效。這些長年存在的漏洞，不僅造成了健保資源的浪費，也導致受保人權利與義務不均衡、醫病關係變得扭曲，甚至使醫療機構無法正常合理經營。這些問題若不好好整治，將日漸腐蝕健保的根基，使制度難以維繫。
亦即，要維護健保制度的健全，是一個全方位、多面向的工作，必須要多管齊下，杜絕浪費，嚴懲不法，持續追逐效率和公平。否則，若僅知一味訴諸調漲費率來維持收支平衡，只是最怠懶的作法。這點，特別要提醒楊志良：別以為僅憑一時英勇，改革就能一步到位。
健保的長年問題，在此簡單列舉數例：一，多年來，北高等縣市積欠健保補助款達數百億，導致健保財務運轉困難，其中有統籌分配款之爭，有制度合理與否的問題，中央及地方政府均有責任妥速解決。政府把問題拖到現在，卻說要調漲費率才能解決，等於把責任轉嫁給民眾，人們怎麼會服氣？
二，越來越多旅居海外台胞返台接受健保醫療，許多人並未在台納稅，其繳交之健保費用亦多屬低估，這難免對國內長年、定時繳費民眾的權益構成侵犯。其中疏漏的公平性，應該設法補強。此外，包括國內許多「高收入、低繳費」的所謂「林志玲條款」問題，如果不能設法克服，都將使健保局宣稱的「劫富濟貧」變成笑話。
三，各級醫療院所虛報、浮報或利用人頭詐領健保費的情形一直存在，據估計，每年金額高達近百億，這不僅是惡性地「吃健保」，更造成許多正派經營的業者難以競爭，導致劣幣驅逐良幣。對此，健保局應該訂出更有效的規範之道，並窮盡各種手段向偽詐者追討回來。此外，對某些以看病、拿藥為樂的民眾，嚴重浪費健保資源，健保局也應該訂出合理的計費辦法，甚至採取盯人勸導戰術，別讓少數人破壞全民共有的制度。
楊志良不畏艱險的精神，值得欽佩，但他絕不能把事情想得太簡單、太天真。所謂每個月「多花一個便當錢」，就可以解決健保財務困境，聽起來是很動人的口號；但別忘了，不僅所有受薪者要多繳錢，所有企業主的負擔也要相對加重，而現在卻不是個好時機。高官一句話，就要民眾多體諒政府；但人民的苦，政府能說自己又體諒了多少？
其實，這回要能成功拔取鵝毛，楊志良並不需要作出鐵漢姿態，他最好能端出全套改革計畫，好好向全民溝通，爭取大家認同。健保制度並不需要一個烈士來殉身，要有手腕讓民眾甘之如飴繳費才行。
