This is the lesson I have learned recently. Caution is never enough. Ask and Ask. Get to the bottom of the question or answer. Dig out the root cause behind those situations. Think broader. I shall have learned from the contract for the PH customer this time. The repetition/ change in the request would only bother customer if I do not get things straight at the very beginning. People will be suffered because of that.
